Grants to USA nonprofit organizations, community-based organizations and coalitions, academic institutions, and medical and professional societies for training and service delivery programs that promote equitable access to health care. Funding is intended to reduce health inequities and improve health outcomes for medically underserved individuals.

Grantmaking and partnership development regarding the request for proposal should focus on community health workers (CHW) and patient navigation (PN) training and service delivery programs that address the following:

  • Community Outreach and Engagement for Linkage to Care & Supportive Services
    • Assist patients to access healthcare coverage and linkage to a medical home
    • Bolster disease and awareness education and increase utilization of screening and preventative care services
    • Provide culturally sensitive and appropriate support for patients that  address social determinants of health
  • Care Coordination and Patient Navigation
    • Focus on continuum of care so patients are not lost to follow up and receive equitable, high-quality care
    • Improve transitions in care and referral between primary care and specialty including improving awareness and enrollment in clinical trials among diverse patient populations
    • Support patient clinical journeys through treatment initiation, maintenance and monitoring inclusive of psychosocial needs
  • Ensuring the cultural competency and ethnic diversity of the CHW and PW workforce
